Country Inn and Suites Credit Card Authorization Form

The Country Inn and Suites credit card authorization form is used to allow third-party expenses to be charged to the credit card in question. A cardholder can, with the use of this document, authorize a third party individual to charge their credit card when staying at a Country Inn & Suites location. It is necessary that a reservation is placed ahead of time using the credit card in question, providing this document does not initiate a reservation. This particular chain of hotels asks that the front and back of the credit card be photocopied along with a piece of photo ID and that both be faxed with the completed document. How to Write

Step 1 – Download the credit card authorization form in Adobe PDF and open it up in your web browser/PDF reader of choice.

 

Step 2 – Read over the introductory paragraph to ensure that you’re aware of what it is you’re authorizing. The fax number and contact number of the Country Inn you’re attempting to contact should be retrieved prior to filling the form. If you’re using a location-specific document, the below fields will be filled out with the contact information already.

Step 3 – The cardholder information must be provided first and foremost. The following data must be submitted into the document:

  • Name as appears on credit card
  • Card type (Visa, MasterCard, Discover, American Express)
  • Account type (Individual or Corporate)
  • Account number
  • Expiration date
  • Address (where statement is mailed)
  • City, State and Zip code
  • Phone number of cardholder
  • Fax number/alternate number of cardholder

Step 4 – The guest information needed is as follows:

  • Guest’s full name
  • Company name
  • Phone number
  • Fax/alternate phone number
  • Confirmation number (may need to call to retrieve this information)
  • Check in date
  • Check out date
  • Relation to cardholder
  • Room rate and taxes (to be filled by hotel)
  • Total daily rate (to be filled by hotel)
  • Number of nights

Step 5 – Read over the final paragraph stating that you certify that all information is accurate and that you authorize the hotel to charge your card. You can put a maximum amount that the card can be charged within this paragraph if you wish. Print off the document, provide your printed name and signature, and then send the completed form to the applicable hotel location via fax. It must be accompanied by a photocopy of the card in question and a photocopy of a piece of photo ID.
