The Howard Johnson credit card authorization form is a very basic document which allows for the pre-authorization of the use of a credit card by an individual other than the cardholder. This document is most commonly used when an employee of a company is using a company credit card to pay for a hotel room. It is not uncommon for an individual to show up at reception anticipating that the card which was used to make the room reservation can also be used to finance their stay; this is not the case. A credit card authorization form must be completed and faxed/emailed to the appropriate Howard Johnson location prior to the guest’s stay to enable the front desk to authorize the use of the card in question.

How to Write

Step 1 – Download your form in Adobe PDF and open it up in a PDF reader. You will find that it is fillable online already, however, if you wish to print it off and complete it by hand, you may.

 

Step 2 – Begin by entering in the following information:

  • Guest name
  • Company name
  • Confirmation number (call the hotel in question to receive this piece of information)
  • Date of arrival and departure
  • Credit card type

Step 3 – The credit card number to be charged for payments must be entered into the first field here. Next, supply the expiry date and the CVV code (found on the back of the card). The cardholder’s name and full address should be supplied along with their contact information before proceeding to the next step.

Step 4 – Leave the first field blank here as it is for the hotel to fill in. However, you should provide the date and indicate the services which will be charged to the credit card. When ready, print off the completed form and sign it on the “CARDHOLDER SIGNATURE” line. Fax or email the document to the Howard Johnson location in question along with a photocopy of a valid piece of photo ID and a clear copy or photograph of the credit card indicated above.
