The M&T direct deposit authorization form enables an individual to have funds placed into their account automatically and electronically. This is advantageous as it results in a much quicker access to the payment, in fewer trips to the nearest branch, and in opportunities to deposit directly into a savings account thus instigating a savings plan. The form is simple, it requires only a few minutes and very limited information from the payee. If this document is being used for the direct deposit of paychecks, the employer or payroll department should be contacted prior to completion to ensure that no other forms will be required to initiate direct deposit.

How to Write

Step 1 – Begin by downloading the document in Adobe PDF to prepare to fill it in on your computer. Alternatively, you may enter the necessary information with black or blue ink.


Step 2 – The company name or employer’s name should be filled in at the top. The primary account information should then be entered in the first box. This will include the type of account, the account number, the routing number (see full list linked above), and the deposit amount. You have the option to deposit the installments into multiple accounts; if you wish to add others, continue onto step 3.

Step 3 – Enter in the same information you did for the first account for every other applicable account. If you plan on depositing all of the income into a singular checking or savings account, leave this section blank.

Step 4 – The below personal information is vital. Be sure to enter all of it into the applicable fields before printing off the document and signing it.
