The Inuit / Quickbooks direct deposit authorization allows employees to request direct deposit from their employers. The form presented and linked below must be completed by the employee and handed into the applicable payroll department to be kept with their files. Along with the completed form, it may be necessary to attach a voided check where indicated on the document. Receiving income immediately and directly into a checking or savings account eliminates the time spent depositing a physical check and waiting for it to clear.

How to Write

Step 1 – The form should be downloaded in Adobe PDF and opened up on your computer.

 

Step 2 – The form starts out by asking the employee to provide the information pertaining to the principal account into which the funds will be deposited. Enter in the following:

  • Account type (checking or savings)
  • Bank routing number
  • Account number
  • Percentage or dollar amount to be deposited

Step 3 – The next section is optional and applicable only if the funds are going to be divided into two accounts. If this is indeed the case, enter in the same information as you did in step 2. If you plan on depositing your paycheck into one sole account, leave these fields blank.

Step 4 – Supply a voided check for each applicable account.

Step 5 – Read over the final paragraph to ensure you’re 100% sure of what it is you’re authorizing. Next, enter in the company name in the top field, your employee ID, your full name, and the date of completing the form. Print off the document, sign and give to your employer or payroll supervisor for processing.
