Days Inn Credit Card Authorization Form
The Days Inn credit card authorization form is a form which allows the card holder to authorize the use of their credit card for the purposes of paying for a hotel room. A document of this nature is required prior to the guest’s check-in time to provide proof that the company or individual who is responsible for the card is condoning its use by the guest. It is a common misconception that when a card is used to reserve a hotel room, the guest will be able to use it to pay for their stay and any other amenities used. How to Write
Step 1 – The first step is to download the document in Adobe PDF. Open it up in your preferred web browser or PDF viewer.
Step 2 – The first information that will be demanded of you is the name of the hotel guest(s). Following this, you must submit the following info:
- Reservation confirmation number(s) (you may need to call to retrieve this information)
- Name of Cardholder
- Billing address
- Phone number of Cardholder (+ext.)
- Fax number of Cardholder
- Email address of Cardholder
Step 3 – The Days Inn allows the cardholder to specify exactly what will be charged to their credit card. Choose from the following:
- First night’s room and tax
- All room and tax
- All incidental charges
- Banquet deposit only
- All banquet charges due
If you chose “other”, you will need to leave a comment to define exactly what it is your card can be charged for. The last for digits of the credit card, expiration date, and your signature will need to be provided next.
Step 4 – Here you will need to specify the type of credit card before submitting your credit card information (number, expiration date, cvv). Fax the completed and signed form to the Days Inn location at which you have a reservation to provide your official authorization.
