This automatic payment system enables the payee to avoid waiting for a check to clear; direct electronic deposits arrive on average two (2) days sooner. How to Write
Step 1 – Download the form in PDF to fill out on your computer or by hand.
Step 2 – The first box has you choosing the amount to deposit into the account. You can choose the entire check, a set percentage, or a set amount. Select your preferred box and enter whatever info is necessary.
Step 3 – The second box has a field into which you must enter your full name and address. The other field is for your direct deposit ID number.
Step 4 – Enter the date into the second field of the third box. Read over the direct deposit authorization statement, print off the form, and sign it. You can then give the form to the proper authority for processing.
