The subject must consent to the use of their image, their name, their likeness, and any statement made in whatever format the organization chooses to present them. The consent given in this form requires no prior approval once the document has been signed, that is to say the organization will not need to check back with the subject to ensure that they agree with the use of their name, likeness, image, or voice.
How to Write

Step 2 – Enter the name of the legal entity/organization to whom you’re giving the authority.
Step 3 – Enter your name here along with your address and the day’s date. Print the form and sign it.
Step 4 – If the subject is a minor child, a parent or legal guardian must sign and provide their name as well. The organizational representative must sign and give the date to indicate that they have read the consent form to the parent/guardian prior to having them sign it.
