The HIV patient consent form is a document that is used to prove that an individual receiving an HIV test has been fully informed of the details pertaining to both the test and the disease. Furthermore, it allows the healthcare provider to continue to perform tests after the principle test has been carried out using the initial sample. This will be done to determine the best treatment for the patient. The consent can be revoked at any point through writing or orally.

Once you’re confident in the test and the ramifications of a positive result, your name, date, and medical record number should be supplied.
